1. Rate available to practice-based physicians and dentists that do not currently accept American Express Cards. Acceptance is subject to the terms of the American Express Acceptance Agreement.
2. If you already accept credit card payments, there is no additional equipment needed and no set-up fees will be charged by American Express but terminal providers may charge these fees. Your practice must submit one American Express Card transaction to activate your account.
3. Offer valid only if you do not currently accept credit cards and if you agree to start accepting American Express Cards. American Express will waive the $25 monthly rental fee for one year. You can include additional credit card types at no cost within the first 30-day period. A $25 per card type service fee applies thereafter. Paper is sold separately. Prices are subject to all applicable sales tax. To activate your terminal to accept Visa and/or MasterCard transactions, please contact American Express.

5. Offer available for practice-based physicians and dentists, who do not currently accept American Express Cards. Acceptance is subject to the terms of the American Express Card Acceptance Agreement. The practice must sign up to accept American Express Cards, activate the merchant account and must mention that they heard about this offer from Galaxy and that they are a Galaxy provider, to receive the merchant account credit. The practice must submit one American Express Card transaction to activate. One merchant account credit per each new merchant account (not for each medical professional within the practice).
